OTHER TERMS & CONDITIONS:

1. I have been informed of, understand, and am fully aware that strength, flexibility, and aerobic exercise, including the use of equipment, are potentially hazardous activities. I have also been informed of, understand, and am fully aware that fitness activities involve a risk of injury, including a remote risk of serious disability or death, and that I am voluntarily participating in these activities and using equipment with full knowledge, understanding, and appreciation of the dangers involved. In consideration of being allowed to participate in the personal fitness training activities and programs of The Power to Pause, LLC and to use their facility, equipment, and services, in addition to the payment of any fee or charge, I hereby agree to assume and accept any and all risks of injury or death. Consequently, today and on all future dates, I, for myself, my heirs, personal representatives, executors and administrators, do herby waive and release any and all rights and claims for damages against the owners, officers, operators, employees, trainers, instructors, and other members or representatives of The Power to Pause, LLC, as well as waive and release The Power to Pause, LLC (the “Releasees”) from any and all claims or liabilities for injuries or damages to my person and/or property now, or in the future, foreseen or unforeseen, including those caused by negligent acts or omissions of any of the Releasees or others acting on their behalf, arising out of or in any way connected with my participation in any activities, programs, or services of The Power to Pause, LLC, or by the use of equipment at The Power to Pause, LLC.

2. I understand that this Agreement and Release of Liability will remain in force and effect throughout the duration of my relationship with The Power to Pause, LLC, regardless of whether or not I train on a continuous basis.
